The care network around you
A nursing practice replaces neither emergency services, nor the health insurance system, nor social services. Here is plainly who to call in each situation — in our sector and across Isère.
This page names no private practice, laboratory or doctor: the nursing code of conduct forbids promoting colleagues, and an unverified number has no place in a care context. We therefore list only public institutions and official services, each with its source.
Emergencies and out of hours
A nursing practice is not an emergency service. Here is who to call, and in what order.
SAMU — 15

Coordinating care at home
The organisations we work with when a situation goes beyond clinical care.
DAC Sud Isère (GCS MRSI)
IsèreCare coordination support: when a situation becomes complex — several providers, carer exhaustion, a difficult hospital discharge — it brings everyone around the home together. We call on them ourselves when it helps.
Saint-Martin-d'Hères, working days
CHU Grenoble Alpes
IsèreThe area's reference hospital. Its discharge teams pass on the prescriptions and reports that home care depends on.
Département de l'Isère — autonomie
IsèrePersonal autonomy allowance, home help, housing adaptation: this is where the support that makes staying at home sustainable is applied for.
CCAS d'Échirolles
SectorMeals delivery, personal alarms, social support for older or isolated residents in the town. The first port of call when the problem is daily life rather than nursing.
CCAS de Grenoble
SectorSame role for southern Grenoble: home help, meals, social connection, access to entitlements.
Rights and cover
What belongs to the health insurance system and patients' rights, not to the nurse.
Assurance Maladie — 3646
NationalHealth card, long-term condition status, full cover, reimbursements, registered GP. The online account gives the written answer a phone call cannot.
local call rate
Santé Info Droits — France Assos Santé

3977 — maltraitance des personnes âgées et handicapées
