Privacy policy

This site has no patient portal, no accounts and no health database. The only data processed is what you voluntarily enter in the contact form.

Data collected

• Full name
• Phone, email (optional)
• Town
• Free-text content of your message

Purpose and legal basis

This data is used solely to contact you back about your request. Legal basis: your consent, given via the form checkbox.

Health data

The form is not a secure health-data channel: do not enter medical information there. Clinical information is collected by phone or during the visit, and stored in the practice's professional software hosted by a certified health-data host.

Retention

Requests received through the form are deleted at most 12 months after the last exchange, unless care has started.

Processor

The form is routed by web3forms, for one purpose only: routing the contact form to the practice's mailbox. No data is stored on this site.

Analytics

No advertising cookies, no third-party trackers. If analytics are enabled, they will be cookieless and hosted in the European Union.

Your rights

You have rights of access, rectification, erasure and objection. Write to contact@example.fr. You may also lodge a complaint with the French data protection authority (CNIL).
